Targeted Case Management services help individuals and families access services that could improve the quality of life. We provide referral and connection to community programs that support basic, medical, educational or social needs. Our targeted case managers offer support to families and education to the individuals involved in the client’s care. They maintain regular contact and meet with families at least twice a month. The clinician will complete an initial assessment through screeners, observation, collateral information, and interview to determine if the child needs this service. They will make a referral for this if it is identified that your family qualifies for this service. Please ask your provider if you believe this may benefit your family.
We can develop an individualized plan of care for your child and assist with finding the resources appropriate for achieving the goals specified in their care plan.
Our services address the areas which may be causing barriers to treatment. The individual provider can work collaboratively with the targeted case manager to offer guidance on client needs and areas of support. The targeted case manager works with the client outside of therapeutic sessions to support overall mental health. The targeted case managers’ interventions support the client’s treatment team to address the clients specialized needs.
